Specimens can be sent to a reference laboratory for assistance or second opinions on Congo red staining or immunohistochemistry. Each laboratory has specific requirements as outlined below. Please contact the laboratory for further information as required.

Tissue block and Congo Red stained slide
If the referring lab cannot release tissue blocks, send the following unstained slides:
2x 4 micron thick unstained slide
8x 7micron thick unstained slides

Document requirements
A copy of the original pathology report
Request for amyloid protein typing using immunohistochemical (IHC) stains
Test specification
Review of Congo Red stained slide and examination under polarised light to confirm the presence of amyloid
Amyloid protein staining using TTR, kappa, lambda and AA IHC stains
Cost Medicare rebated if both a pathologist and clinician request review

Sample requirements
The original H&E and Congo red stained slides plus 8 unstained slides of routine thickness for immunohistochemistry.
Document requirements
Please include:
- A copy of the original pathology report
- The pathologist’s name and provider number requesting/consenting to the second opinion and/or additional immunohistochemistry
- The pathologists’s authorised signature as per the request form.
- The downloaded request form is applicable when the treating clinician and the pathologist (who provided the original opinion) agree upon a second opinion. A Medicare rebate is provided in this circumstance.
Test specification
Review of tissue by Congo Red Staining and examination under polarised light for the presence of amyloid
A standard panel of kappa and lambda IHC and IF, amyloid A and transthyretin IHC is performed, and assistance with referral for further testing for rare amyloid types including mass spectrometry.
